Transforming Longitudinal Care with Digital Biomarkers and Therapeutics

Summary

The promise of individualized, continuously adaptive care has been technically within reach for over a decade, yet its realization remains elusive. This talk presents a vision for digitally-enabled hybrid longitudinal care that combines synchronous and asynchronous clinical encounters with continuous patient-generated data (from wearables, ambient devices, and digital health tools) to shift healthcare from transactional episodic visits toward lifelong, relationship-based management. We examine a layered technology stack in which Digital Biomarkers (DBx) and Digital Therapeutics (DTx) leverage AI to bridge measurements to clinical action, with the goal of making patient-generated data as routine and actionable as a blood test.
While meaningful progress has emerged in selected contexts such as Parkinson’s management, arrhythmia detection, and metabolic interventions, scaling these benefits affordably across more patients and providers remains constrained by payment structures, workflow integration, and insufficient clinical validation, not by the availability of underlying technology. We suggest strategies for accelerating progress: designing hybrid care systems around measurable care pathways and patient-clinician collaboration, building on modular open platforms like JupyterHealth to enable rapid iteration and broad reuse across conditions and settings, and exploring these innovations as care delivery advances that improve patient outcomes and reduce costs, rather than as revenue-focused new ventures, particularly given the robust products already available for integration– wearables, EMRs, and AI/LLM services.

Short bio

Deborah Estrin is a Professor of Computer Science at Cornell Tech in New York City where she holds The Robert V. Tishman Founder’s Chair, serves as the Associate Dean for Impact, and is an Affiliate Faculty at Weill Cornell Medicine. Her research interests are in digitally-enabled innovations that support more patients and providers to improve clinical outcomes and quality of life. Estrin founded the Public Interest Technology Initiative (PiTech) at Cornell Tech, which promotes public impact as a component of students’ training and future careers. Before joining Cornell she was previously the Founding Director of the NSF Center for Embedded Networked Sensing (CENS) at UCLA; pioneering the development of mobile and wireless systems to collect and analyze real time data about the physical world. Estrin’s honors include: the IEEE Internet Award (2017), MacArthur Fellowship (2018), and the IEEE John von Neumann Medal (2022). She is an elected member of the American Academy of Arts and Sciences (2007), the National Academy of Engineering (2009), and the National Academy of Medicine (2019).
